I built this tool as I needed a light weight way to quickly embed a nice looking form into a bunch of client's existing HTML sites (ancient basic sites only and no way to easily change the back end or restart the systems.)
I had it just setup as a back end script originally for a bit, but added a front end to it over the weekend. It allows you to create a form and have it email you directly from the form when submitted. No account needed.
