Cortexa Note
AI scribe that turns patient visits into complete charts
Cortexa Note is an AI clinical workflow platform that captures patient conversations and turns them into structured, reviewable SOAP notes, code candidates, follow-up tasks, and EHR-ready records. Unlike scribes that stop at transcription or summarization, Cortexa Note helps complete the work after the visit. Use the web and mobile apps, customize templates, or add the optional Cortexa Recorder for hands-free capture across exam rooms, rounds, and home visits.
Hi Product Hunt! I’m part of the GTM team at Cortexa Note. My role focuses on speaking with clinicians, organizing product trials, and bringing user feedback back to our product team. I did not work on the engineering or hardware, but I have contributed to how we bring the product to users. I’m excited to share it here and hear what this community thinks.
Cortexa Note is designed for the work that continues after a patient visit. It turns the encounter into structured, reviewable documentation, identifies code candidates and follow-up tasks, and helps prepare records for the EHR. There is also an optional recorder for settings where using a phone is not practical.
New users currently receive 14 days of the Clinician plan automatically after signing up. I’d especially like to hear what would make an AI scribe genuinely useful and trustworthy in your workflow.
